Executive Summary
Evan Richardson is the founder and chief executive of Form Health, a virtual obesity medicine clinic he started in 2019. Form pairs each patient with a care team of a board-certified physician and a registered dietitian, who build an individual weight-loss plan that can include FDA-approved medication. It accepts most insurance and works with employers. Richardson has pressed that distinction as GLP-1 drugs have crowded the market, contrasting Form’s clinical model with competitors that he says hand out GLP-1s or compounded drugs without the care needed for safe long-term results. He came to the business from health technology: he was on the founding team of Grand Rounds, now Included Health, and earlier helped build Castlight Health. Form has raised a $12 million Series A and a $38 million Series B.

Professional Journey
Richardson began his career at Bain & Company and Berkshire Partners. By his own account, working as a healthcare investor drew him to health technology. The first company he examined closely was an obesity care service connected to a major diabetes hospital, and it showed him how much evidence-based medical care could do for people living with obesity.
He moved into operating roles at two companies that helped define the category. At Castlight Health he started as a Healthcare Navigator and later became Head of Enterprise Products. He then joined the founding team at Grand Rounds, a clinical navigation company, and held leadership roles across product, data science, operations, finance and corporate development. Grand Rounds later combined with the telehealth provider Doctor on Demand to become Included Health.
He founded Form Health in 2019 to use technology and new drug treatments to deliver evidence-based obesity care at scale. The first patients were seen in 2020, in a virtual model built on the clinical approach of Chief Medical Officer Dr. Florencia Halperin, co-founder of the Center for Weight Management and Metabolic Surgery at Brigham and Women’s Hospital. The case for telehealth was access: when Form raised its $12 million Series A in June 2021, Richardson said the average Form patient in Texas lived 160 miles from a doctor. At that point Form was available in 21 states and planned to expand to 90 percent of the US population.
The $38 million Series B came in June 2024, led by Sound Ventures, after $26.8 million raised earlier. By then Form said it had treated tens of thousands of patients and had relationships with employers, pharmaceutical companies and patients. Richardson also argued that its patient base was producing one of the largest data sets on obesity care. More recently the company has begun prescribing through a Medicare GLP-1 bridge program and was named to Inc.’s 2026 Best Workplaces list.
Outside Form, Richardson sits on the boards of Bicycle Health, a virtual provider of treatment for opioid dependence, and Crossover Health. He lives in Boston.
